Background and aimsThe metabolic syndrome (MetS) is a cluster of coexisting cardiovascular risk factors. The role of specific dietary fats was reemphasized by dietary recommendations. This systematic review aims to assess evidence for the effect of dietary fat intake on MetS occurrence and reversion in adults.Methods and ResultsThe MEDLINE database was used to search the existing literature. We included observational studies that analyzed dietary fat intake in adults with MetS and clinical trials that compared the effects of different dietary fat diets on MetS and/or its components. Thirty articles were selected (14 observational and 16 clinical trials), and we included information of dietary fat and fatty acids as well as MetS, body mass index, cholesterol, hypertension, and diabetes in adults. SFA intake was found to be positively associated with MetS components. Most of the observational reviewed studies found beneficial associations between MUFA and PUFA (including n-3 and n-6 subtypes) intake and MetS components. Clinical trials also supported the benefits of MUFA- or PUFA-enriched diets (including low-fat diets) in reducing MetS.ConclusionsThe effects of dietary SFAs on MetS will be influenced by other specific nutrients. Replacement of SFA by MUFA and PUFA has been associated with a decrease in MetS. Dietary recommendations should emphasize on different qualities of fat intake, not only to reduce total fat intake, to obtain health benefits in adults. 相似文献

ObjectivesTo describe recent trends in advanced imaging and hospitalization of emergency department (ED) syncope patients, both considered “low-value”, and examine trend changes before and after the publication of American College Emergency Physician (ACEP) syncope guidelines in 2007, compared to conditions that had no changes in guideline recommendations.MethodsWe analyzed 2002–2015 National Hospital Ambulatory Medical Care Survey data using an interrupted-time series with comparison series design. The primary outcomes were advanced imaging among ED visits with principal diagnosis of syncope and headache and hospitalization for ED visits with principal diagnosis of syncope, chest pain, dysrhythmia, and pneumonia. We adjusted annual imaging and hospitalization rates using survey-weighted multivariable logistic regression, controlling for demographic and visit characteristics. Using adjusted outcomes as datapoints, we compared linear trends and trend changes of annual imaging and hospitalization rates before and after 2007 with aggregate-level multivariable linear regression.ResultsFrom 2002 to 2007, advanced imaging rates for syncope increased from 27.2% to 42.1% but had no significant trend after 2007 (trend change: ?3.1%; 95%CI ?4.7, ?1.6). Hospitalization rates remained at approximately 37% from 2002 to 2007 but declined to 25.7% by 2015 (trend change: ?2.2%; 95%CI ?3.0, ?1.4). Similar trend changes occurred among control conditions versus syncope, including advanced imaging for headache (difference in trend change: ?0.6%; 95%CI ?2.8, 1.6) and hospitalizations for chest pain, dysrhythmia, and pneumonia (differences in trend changes: 0.1% [95%CI ?1.9, 2.0]; ?0.9% [95%CI ?3.1, 1.3]; and ?1.2% [95%CI ?5.3, 2.9], respectively).ConclusionsBefore and after the release of 2007 ACEP syncope guidelines, trends in advanced imaging and hospitalization for ED syncope visits had similar changes compared to control conditions. Changes in syncope care may, therefore, reflect broader practice shifts rather than a direct association with the 2007 ACEP guideline. Moreover, utilization of advanced imaging remains prevalent. BackgroundDetermining prognosis in community acquired pneumonia (CAP), is very important. Many scores are introduced up to now for prediction of pneumonia prognosis like SMART-COP.ObjectiveTo evaluate validity of SMART-COP score in prognosis and severity of CAP in emergency department (ED).MethodsAll patients older than 18 years old with clinical suspicion of CAP (meeting the inclusion criteria), were enrolled in our study. In this prospective study, patients were admitted to the ED of a tertiary referral center. Hospital length of stay, rate of intensive care unit (ICU) admission, mortality rate, number of intensive respiratory or vasopressor support (IRVS) use, patients' SMART-COP scores and all demographic data were recorded. Validity of SMART-COP in the prediction of IRVS rate and its correlation with other variables were determined.ResultsIn this study, 47.6% and 52.4% of patients were females and males respectively. The mean age of patients was 68.13 ± 16.60 years old. The mean hospital length of stay was 13.49 ± 5.62 days. Of all patients entered in our study, 55 cases (38.5%) needed ICU admission, 29 cases (20.3%) were expired within 1 month and 44 cases (30.8%) needed IRVS during their treatment. SMART-COP ≥5 (high risk CAP) accurately predicted the rate of ICU admission, one-month mortality and IRVS need (p-value = 0.001).ConclusionsSMART-COP≥5 had a high sensitivity and specificity in the prediction of patients' prognosis with severe CAP in the ED. 相似文献

目的 构建一套科学、系统的护士应用体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)技术的核心能力评价指标,为护士的培养、考核和能力评价提供参考依据。 方法 通过文献回顾、半结构式访谈形成护士应用ECMO技术核心能力评价指标初稿,2021年2月—4月,采用德尔菲法对6个省份的25名专家进行问卷函询,运用层次分析法确定各级指标权重。 结果 共开展2轮函询,问卷有效回收率分别为90.00%、92.59%,第2轮函询专家的权威系数为0.886,肯德尔和谐系数为0.205(P<0.001)。最终构建的评价指标包括临床专业知识、临床实践技能、评判性思维能力、专业发展能力、沟通协调能力、管理能力、个人特质7个一级指标、19个二级指标、70个三级指标。 结论 构建的护士应用ECMO技术核心能力评价指标具有较高的科学性和可靠性,对护士的培养、考核和能力评价具有指导意义。 相似文献

目的 调查安徽省4所综合医院临床护士的营养护理能力,并分析其影响因素,为提高临床护士营养护理能力提供理论依据。方法 采用便利抽样法,2021年1月—2月应用一般资料问卷、临床护士营养护理能力量表对安徽省北部、中部、南部4所综合性医院的863名临床护士进行横断面调查。通过多元线性逐步回归分析临床护士营养护理能力的影响因素。结果 共回收有效问卷762份,临床护士营养护理能力总分为(235.16±44.02)分,条目得分为(3.73±0.70)分,其中营养专业知识维度和临床营养评估能力维度得分较低。多元线性逐步回归分析结果显示,工作年限、科室、定期接受营养护理督查、在校学习营养课、参加营养学习班、定期参加营养业务学习是临床护士营养护理能力的影响因素(P<0.05)。 结论 临床护士营养护理能力有待提高,护理管理者应重视对临床护士进行营养相关教育及培训,强化营养专业知识和临床营养评估能力,完善定期检查与考核机制,以提高临床护士的营养护理能力。 相似文献
